How to request transcripts from Mar Vista High School:

  • Fee: Free
Mar Vista High Graduate

In Person
  • Year of graduation and name used while attending high school.
  • Date of birth.
  • Address where you want transcript mailed.
  • ID with picture
  • Fee:$2.00 payable in the MVH ASB
***Please allow a 3 day turn around***

  • Fee: $2.00 per official​ transcript - mail in cash or money order
​*Agencies requesting proof of graduation for pending employment must fax the signed release form for the applicant to (619) 424-8998.*

Work Permits
Statement of intent to employ a minor and request for a work permit – certificate of age

Process for SUHSD students to obtain a Work Permit
  1. Download the SUHSD Form No. B1-1 (intent to employ form.)
  2. Complete the SUHSD Form No. B1-1. Make sure it has BOTH employer AND parent signatures and information, and your social security number.
  3. Take complete SUHSD Form No. B1-1 to your school site. (During Distance Learning, please email to [email protected])
  4. Work Permit designee will review the form as well as your attendance and academic records (must have 2.0 GPA and higher.)
  5. Your Work Permit will be completed within 3-5 business days if you meet district attendance and academic requirements.
  6. Pick up your work permit, SIGN and DATE it, and take it to your employer. (During Distance Learning, work permit will be e-mailed.)
The employer MUST keep your work permit at the work site.
Work permits must be renewed each year at the start of school, or if employment changes.
